When making your appointment with North Jefferson Imaging, the scheduler will prompt you to visit this page.  

 

Simply click on the links that are discussed with you, Print them out and fill them out.  Bring the forms with you on the day of your appointment to save some time. 
 

If you have any questions about the form or you are not sure about how to complete some portions, leave them blank and our friendly staff will assist you during your visit.
